Service Agreement

This Service Agreement is for clients of the Department of Human Services (DHS) Disability Services and a decision-maker on your behalf if you do not have capacity to enter into a legal agreement.

The Service Agreement is made up of 3 parts.

Parts 1 and 2 are unique to each client:

  • Part 1 – Party Information
  • Part 2 – Service Summary.

Part 3 applies to every client.

The Service Agreement is a legal agreement that outlines how services will be delivered and the rights and responsibilities of both clients and DHS.

This Service Agreement is subject to change.

Part 3 - Terms and conditions

The service agreement outlines the terms and conditions of the services DHS delivers.
