Transition to Home

Transition to Home provides short-term housing and support for eligible NDIS participants who are ready to leave hospital.

Transition to Home allows people with disability to move out of hospital and into a more home-like environment while they prepare for their long-term home.

The service creates more capacity in hospitals and supports people with disability to regain their independence and rebuild important connections with their community.

We have Transition to Home services at St Margaret’s Centre, Semaphore and near Noarlunga Hospital.

We also have 5 transitional units for NDIS participants with extremely complex support needs who require a more specialised setting.

Support at Transition to Home is similar to our Supported Independent Living service. Support workers support each person with their personal care, meal times and daily routine.

Transition to Home is a collaboration between the Department of Human Services, Department of Health and Wellbeing and Local Health Networks. If you are in hospital, you can talk to your doctor about being referred to this service.
